Question about the author information for your entry.

When it finally comes time to submit, do you put your contact info in an entirely separate document, or do you begin your manuscript's pagination with zero, putting contact info on page 0 and starting the story on page 1?

I started the story with pagination on 1, title on every page, and made sure not to have any identifying information as to my name anywhere in the document. When you submit electronically, you'll fill in a box form with all your contact info that will serve as your contact/title page. (someone correct me if I'm wrong, last time I submitted was October, and I'm trying to wrap up a novella for submission this week before I get my Q2 edits finished and sent in myself)

Blind submission means no contact info on any pages of your manuscript, not on the first page, not on the second, not in the headers, not in the file name. You will be immediately disqualified if you leave it on. When you fill out the Wizehive form on the submission portal, that's where you fill in your contact info.
